Мechanism of action

Another name for isotretinoin is 13-cis-retinoic acid. It belongs to the retinoid class and is synthesized from vitamin A. How does Accutane work? Although the exact mechanism of action of isotretinoin could not be determined, several studies have shown that it causes programmable death (apoptosis) of different cells.

In the case of acne, the death of sebaceous gland cells occurs, which helps eliminate rashes. The drug also has antimicrobial effects on Cutibacterium acnes bacteria that affect the skin.

Isotretinoin is the only remedy stopping the main pathogenic processes that trigger acne development:

  • the drug reduces skin fat production
  • skin renewal is accelerated
  • it eliminates an environment conducive to bacterial reproduction

At the same time, the concentration of the drug in the blood is 1.7 times lower since it does not penetrate the red blood cells.

Metabolization occurs by forming three metabolites: 4-oxo-isotretinoin, tretinoin, and 4-oxo-tretinoin. The half-life of the terminal phase is 19 hours. The drug is excreted by the kidneys and liver together with urine and bile.

Accutane dosage and intake characteristics

Accutane is available in capsules of:

The dosage is calculated based on the formula: 0.5-1 mg/kg of weight. Usually, it is divided into two doses per day. In the course of treatment, the dosage can be adjusted according to the results. In severe cases, it is increased to 2 mg/kg.

Since the drug is better absorbed with food, it is recommended to take pills while eating. Taking it once a day is not recommended.
